In-Network Insurance

We are in network with the following insurance companies:

We can directly bill your insurance and your co-pay and authorized visits will be determined by your plan with your insurance company.

Each insurance company has different policies for physical therapy.  It is strongly recommended that you review your benefits information. Any questions regarding coverage should be directed to your insurance carrier directly. Please bring all insurance cards and therapy prescriptions with you to your first appointment. 

Scripts will be required after thirty days according to NYS Direct Access laws.

  • Blue Shield of NENY

  • Blue Cross (Anthem, Empire, Federal, Medi-Blue, & Forever Blue)

  • MVP (Cigna)

  • Medicare (requires script) 

  • Worker's Compensation

  • No Fault

  • CDPHP


Out-Of-Network Insurance

Although we are out-of-network with certain insurances, you are still able to submit for out-of-network benefits.

  • Tri-care

  • NYSHIP/Empire Plan

  • Aetna

  • United Health Care

"Out-of-network" means that you will pay our self payment rate, but are able to submit the bills to your insurance company for reimbursement.


Non-Participating Insurance

The insurances listed below are not accepted and do not allow for out of network benefits.  We do, however, offer private pay prices.

  • Medicaid

  • Fidelis

  • Wellcare

With these insurances, we cannot submit billing. please refer to our self-pay rates for session prices.


Self-Pay

Our self- pay option is available to you if we do not participate with your insurance, you do not have insurance, or you go over the number of authorized visits allowed by your insurance plan.

Listed below are the prices for our self-pay option:

20 minute session: $65.00

40 minute session: $115.00

60 minute session: $165.00

A script will be required after thirty days from your first appointment.  Direct Access is available.  Direct Access means that for therapists who qualify, you can be seen for physical therapy without a script for 10 consecutive visits or 30 days, whichever comes first.  After that time period, a script must be obtained, even if it is not required by your insurance company.  This is NY state law.


Workers Comp/ No Fault

we do participate with workers compensation, however, coverage may be dictated by the following:

  • If you have had prior therapy

  • Your insurance carrier

  • A script, variance, or referral may be needed

  • The date of the injury

  • An active workers compensation claim

we also work with No Fault, although the following will be require:

  • A new script every 45 days

  • Your no fault carrier information

  • A claim number and date of accident

  • An active claim
